Required information:

1. English application form (original received)

2. 1 recent passport size photo (original received)

3. Passport (original and photocopy, all non-blank pages are copied, and the last page is signed by myself to receive the original and photocopy).

4. Book a round-trip air ticket (the entry date and departure date on the application form must be the same as the reservation form, and the original should be received).
