How to fill in the tableware disinfection record form

1. Header information: at the top of the table, fill in basic information such as date, time, relevant department or person in charge.

2. Tableware list: list all kinds of tableware used, such as bowls, plates, chopsticks, etc., and indicate the quantity of each.

3. Disinfection operation record: fill in the relevant contents of each disinfection operation line by line according to the date and time sequence.

4. Signature confirmation: the last line is left blank, and the person in charge will sign and note whether it is in compliance after each collation and verification.
