How to fill in the Singapore entry card?

Please follow the attached drawings, from left to right and from top to bottom:

Left column:

English name and gender on passport (tick) Male, female.

passport number

Passport issuing place, valid date and place of birth.

Date of birth: year, month and day

nationality

Malaysian Identity Card Number (not required)

flight number

Middle column:

Permanent residence: city, province and country

The last port of departure before coming to Singapore (country)

The next port of entry after Singapore (country)

The address of Singapore

(Scheduled) Days of Stay in Singapore

Have you been to Africa or South America in the past six days (check: Yes, No)?

Have you ever used a passport with a different name to enter Singapore before (tick: Yes, No)?

Have you ever been banned from entering Singapore (tick: Yes, No)?

English name on passport (in English capital)

nationality

Malaysian Identity Card Number (not required)

If you don't leave Singapore and transfer directly, you don't have to fill in the new address.

Next time: Even if you plan to stay in a hotel in Singapore, you generally don't have to fill it in.
