Which is better, one day off in the day shift, one day off in the night shift and four days off in the month?

When nucleic acid becomes a part of daily life, time is counted in hours for nucleic acid testers.

The resident population in Beijing is 2 1 0.886 million. If it is tested once a day in a mixed way of1,it needs at least 210.80 million tubes a day. According to the regulations, each nucleic acid testing institution needs to issue the test results within 24 hours after sampling, and some institutions put forward higher requirements for themselves-the system will issue an early warning after 22 hours.

The amount of inspection is huge and the time is extremely short, which means the demand for a large number of inspectors. Every institution tries to attract inspectors from all over the country with high salaries and benefits. In the recruitment group of third-party organizations, the daily salary 1500, accommodation, car delivery and payment of six insurances and one gold have already become standard. On the other hand, the standards of recruiters have to be lowered again and again. An unlicensed "expert" who is "familiar with the process and proficient in more than two experimental steps" can take up his post the next day after the online interview.

There have also been some chaos in this industry. On May 14, the supervisor found that the original test data of Piaoshi Medical in Fangshan District of Beijing was obviously less than the number of samples. Subsequently, Fangshan District Health and Health Commission revoked the practice license of Piaoshi Medical. A few days later, the license of Beijing Jinzhun Medical Laboratory was also revoked, because it illegally tested multi-tube samples, which seriously violated the quality and safety regulations and affected the accuracy of the test results.

Zhao Jian (pseudonym) graduated from a key medical university majoring in laboratory, and worked in a 3A hospital for nearly 3 years. He came to Beijing to visit relatives on May/KLOC-0. By chance, he worked as a part-time nucleic acid detector.

He was as busy as a war. At most, he tested 16 hours continuously. But for him, the work intensity is passable, and the most uncomfortable thing is that the industry has become "chaotic".

He described how the nucleic acid laboratory works and some phenomena he saw that puzzled him. The following is his dictation-

Operation chestnuts

1 my major in medical university is laboratory medicine. After graduation, I successfully joined a third-class first-class hospital as an inspector, and my daily job was to process samples. Under normal circumstances, the workload is not large. I saw that the number of people actually tested in our hospital yesterday was 6340. After ten mixed samples, 634 samples were processed. The monthly salary can be in the early 1 10,000 yuan. I am very satisfied.

In fact, it is a complete mistake to go to Beijing for part-time nucleic acid testing.

I came to Beijing to visit my relatives during the May Day holiday. As a result, the day after I arrived, Beijing announced that it would stop eating in class. I know the epidemic is serious. If you go back directly, you don't know the isolation cost and time cost, so you might as well stay in Beijing.

A friend here saw me coming and told me that many third-party organizations are now recruiting part-time nucleic acid testing personnel, and there is an urgent need for dual-certificate personnel (testing personnel and clinical gene amplification personnel).

Basically, you can earn 120 yuan/hour, undocumented 100 yuan/hour, and work once a day 12 hours, so you can earn 1440 yuan. I quickly applied to the hospital for unpaid leave. Making money is better than waiting here.

On the first day of work, after entering the laboratory, the laboratory leader will ask me to provide my ID card, mobile phone number, inspector's certificate and PCR certificate issued by the provincial clinical laboratory center (post certificate for clinical gene amplification personnel). In fact, I just went in with the feeling that I could make money every day. No five insurances and one gold, no contract signed, and the salary will be settled the next day.

After I started my part-time job, I also participated in some recruitment groups for PCR nucleic acid detection, and the intermediary sent out recruitment needs every day. In fact, at the beginning, it was not required that the inspectors must graduate from the inspection major or have a college degree or above, which was a new requirement put forward after the accident in Park Stone Laboratory. Part-time nucleic acid sampler generally needs a nurse's card, and part-time nucleic acid information recorder has no special requirements, as long as it is healthy.

However, the information sent by the intermediary should be carefully identified, and many of them are unreliable. For example, an agent said that inspectors 150 yuan/hour and daily salary 1800 yuan could be offered. Fangshan Pushi Laboratory attracted many people with this salary, and everyone saw it.

An intermediary said that 3000 yuan and 4000 yuan can be used for temporary PCR certificates. It is out of the question. If candidates want to get a PCR certificate, they need to register with a hospital laboratory with the qualification to carry out PCR experiments, not any hospital. The receipt after registration needs to be stamped by the director of the hospital laboratory.

In addition to the theory, the qualification examination for nucleic acid testing should also test the candidates' operational ability. The provincial clinical laboratory center will provide blind samples. If positive quality control is added, the samples will have positive, strong positive and weak positive reactions. Finally, the clinical testing center will check whether the positive samples selected by the candidates are consistent with their pre-test settings.

Therefore, this is a very high threshold work, and the inspectors need to understand the principles and steps of the experiment, which can never be completed by any one person.

Compared with clinicians, clinical laboratory specialty is not popular. Most people who study laboratory didn't know exactly what the examiner was going to do at first, mostly because the first choice in clinical practice was not accepted.

I didn't.

I filled in the test as my first choice. At that time, my college entrance examination score was 597, which was enough for clinical study, but I still took filling in the test as my first choice. I'd rather face samples than people. People are too troublesome. The sample is very pure. As long as the test results are correct, no one will bother me.

I got the PCR certificate in 20 19. At that time, before the epidemic started, I was doing other kinds of molecular experiments in the hospital, and I felt particularly relaxed. Who knew I was suddenly so busy now?

Now the laboratory is scheduled every day, each shift 12 hours. I've been working the day shift recently, and I'm on duty in Zone 3.

The whole nucleic acid laboratory is divided into three areas. The first area is to prepare the reagent, the second area is to add the sample to the reagent, and the third area is to enlarge the sample and judge the result.

The so-called "quick hand" required by the intermediary group refers to the speed of adding samples. After screening in Shanghai, it is generally required that the sample loading of 1 hour should be no less than 4 plates (96 tubes/plate). I'm fast, I can fill a plate in 7 minutes, and I can add 9-1 hour. Most experienced people need to add 1 plate 10 minutes or more.

At present, all the big screens in Beijing are undertaken by third-party organizations, and the laboratories of my organization are also distributed in various regions, and each laboratory updates its requirements in the work group every day. If the sample size of large screen in individual areas is too large, part-time staff like us will be mobilized to temporarily cover it for three to five days.

The most exaggerated one was that I was temporarily transferred to the square wooden house in Fengtai District. I went in at 4 pm the day before and worked until 8 am the next day. I have been testing 16 hours, no matter how many tubes I have made. Because I have to screw the specimen cover 16 hours, my fingernails are red at the end of the day and my arms can't be lifted.

Many times, when you enter the laboratory, you will see the dawn, and when you come out, it will be dawn. My concept of time and space has become blurred, and I feel still in the laboratory when I go back to rest.

Usually it's 4 o'clock in the morning after the night shift. I was too tired to go home, so I went to a hotel near the laboratory to sleep. Because hotels are also cheap during the epidemic, 1 19 yuan for one day.

Once when I came back to the hotel from work, my roommate asked me to help him hang his clothes in the closet. I blurted out, "I have frozen the remaining half plate of reaction liquid in the freezer." At that time, my mind was full of experiments and I was fascinated.

Even under normal circumstances, I have to handle at least 15000-20000 tubes of samples per shift, which means 15000-200000 people. At least 1250 tubes of samples should be processed every hour, and the rhythm is to keep working, just like fighting. Working in minutes, it is impossible to fish. The team leader will regularly follow us to count the boards.

If the sample size is large, 12 hours can't leave work, and it takes 13 and 14 hours on average every day. The time for eating in the middle is not fixed. The team leader looked at the progress of the test and said that we can't eat until we finish eating. Because you need to take off your protective clothing when you have a meal, you should drink as little water as possible. What if you are thirsty? Hold on. I am a boy, but it is more troublesome for girls to go to the toilet.

I never thought this industry could make people sick before. Later, I participated in the big screen and did experiments for too long. Everyone is prone to scapulohumeral periarthritis, and finger joints and muscles are prone to deformation and effusion after long-term use.

There will always be various emergencies during screening. Mismatching of personnel information often happens. I scanned the test tube and sent the results, scanning the corresponding position of each sample, but when uploading, the system will feed back a sample without corresponding personnel information. In the mixed sample, as long as one person's information is wrong, the system will prompt that the corresponding health treasure cannot be found. I need to contact the community and find people who match the sample.

Residents are also very anxious to push their grades. The telephone number of the laboratory is published at the nucleic acid detection point. It may be just five hours after sampling, and some residents call the agency customer service to urge the results. Customer service will come to the lab immediately to inquire about the progress. Don't talk every time you ask. I don't know what to say. I also understand that residents may have bulletproof windows or other urgent needs, but I really can't walk fast.

The shelter is also running at full capacity.

In fact, the hardware configuration of the laboratory shelter is ordinary sample flux, but now almost all samples must be Qualcomm. Generally, the flux is 10000 tubes, but basically the sample size received is about 20000 tubes.

Fortunately, there is a sample release solution in my laboratory, and the RNA in the sample can be released automatically without re-extraction.

In this case, the equipment can basically keep up. The sample needs to be heated by the amplifier before it can replicate itself quickly, which is convenient for inspectors to check the sample results. An amplifier can handle 90 samples at the same time. However, if there is no such reagent, the extraction step is still needed, and the experimental time is long, and the results may not be achieved within the specified 24 hours.

Sometimes I am also very upset. It's almost time to get off work, and I've almost packed my reagents. At this time, the people outside just said, "No samples!" I just took a breath. Two minutes later, I opened the door and threw in a bag of samples (90 tubes). Then I can't help it, I can only say it after the exam.

In fact, I have long been trained by this kind of work intensity.

202 1, when the nucleic acid screening was not delivered to a third party, I was more tired in the hospital than now. Sometimes hundreds of thousands of people's samples are completed in one shift, and there is no partner at all. I need to walk the whole course and three districts by myself. From reagent preparation, sample addition, amplification to results, I am the only one. Under normal circumstances, I have to work 14 hours every day.

I worked part-time in Beijing for more than 20 days and found a positive environmental sample. I read it and immediately reported it to the CDC. I'm used to positive samples.

At the worst of the epidemic in 2020, I joined the medical aid team in Hubei. That year, Wuhan was closed during the Spring Festival. By March, all provinces were looking for support from people with nucleic acid testing certificates all over the world. Every third-class first-class hospital has taken the indicators, and those with certificates have to be sent to Wuhan. The atmosphere is particularly tragic, and it is stipulated that pregnant children and only children have to go.

At that time, there were fewer people doing nucleic acid testing than now, so the work intensity was great. We all have no working hours, just keep working, keep working,/kloc-we will continue to work after 0/6 hours. Until someone takes over. If you really don't have hands, keep doing it.

The first time I was on duty in the third district, I saw a positive result and my legs were soft. In the second half of the month, I saw positive results every day. After a long time, I became desensitized.

I just came home from Wuhan in April. Since then, I have not felt tired because of any work. Frankly speaking, compared with 2020, this kind of work intensity is relaxed.

In fact, the work intensity is passable, and what makes me most uncomfortable is that this industry has become particularly chaotic.

On the first day of my part-time job, I walked into the second department of the laboratory and asked my buddy, "What's your major?"

As a result, he never learned the test and often ran Didi. He was able to enter the laboratory only because he had the experience of transporting samples to the sampling point before, and he happened to know the intermediary and entered the laboratory. The driver is not unusual, and there is a real estate agent standing next to him.

When recruiting in the laboratory, it is said that every employee has a probation period, but he doesn't understand the experimental principle. Those who test nucleic acid for the first time can learn while watching others operate. After they are skilled in adding samples and rotating plates, they can cope with the work in the second district. A shift in the laboratory needs at least 7-8 people. Now, any institution or laboratory is short of people. Usually don't really blow him away.

People with recruitment authority will also attract acquaintances and relatives. Our team leader is like this, and his character is very problematic.

He is strict with us, but he protects his relatives well. In order to recruit this relative, I turned down a very capable colleague with double certificates. But this relative really knows nothing.

Once he picked out the pollution when picking samples, and added positive quality control to the samples, and the samples became positive. The person on duty in the third district may judge that the result is false positive, and the sample will be returned to the second district for re-inspection, so I have to start all over again-add the sample again and enlarge it with a computer.

There is another time that is even more thrilling. After re-inspection, I asked him to destroy the samples before 39 sets. As a result, he misunderstood and dumped all the samples after plate 39. Those samples haven't come out yet. If the sample needs to be retested, I can't find the corresponding position. Finally, I can only judge that the weak positive of a sample is caused by pollution according to my own experience, so I can't re-examine.

▲ Figure/People's Vision

Sometimes, I found the whole "flower board" in the third area (the whole board is polluted), and it may be that the positive quality control shook into the board, causing the whole board to show positive results. The whole circuit board should be re-sampled, re-installed and re-measured.

I was so angry that I yelled at him. As soon as he twisted his face, he complained to the team leader that my bad attitude made him unable to work.

Maybe I'm getting impatient. But I really can't accept coming to the lab without knowing anything. We all do it for the money, but there is no escape. No matter what experiment is done, the experimenter should understand the experimental principle, which is the most basic.

Take my organization as an example. They have two kinds of reagents, one is the treatment liquid and the other is the reaction liquid. When the sample is added to the treatment solution, there will be a preliminary reaction, and then it is necessary to turn over the plate and add the reaction solution, and the sample will automatically release RNA. If there is no reaction solution, the experimenter needs to manually extract RNA from the sample. The whole experiment will become very complicated, and each step may pollute the sample, leading to the occurrence of "false positive" of the sample.

I lost my temper because I didn't want this industry to become so mixed up, not because I was abused by laymen or my personal interests were affected.

I worked in Beijing for 20 days and got 30 thousand yuan, which is equivalent to three months' salary in the hospital. After the recent inspection of these nucleic acid testing institutions in Beijing, people with dual-certificate qualifications will receive higher treatment. Institutions need to rely on high salaries and benefits to gather these professionals.

I won't really quit my job to do a nucleic acid test. I'm not that tiger. Public hospitals are worth staying in. Although the money earned every month is small, it is stable. There is no overtime pay for overtime work, but the application for leave can be approved every time. What's not satisfactory?

This part-time job will probably end in a few days. 1 minute ago, the news pop-up window said that Beijing's social screening was zero and catering should be resumed. Like everyone else, I look forward to the day when I can return to my normal life.
