1. Original and photocopy of ID card;

2. Original and photocopy of the application form for motor vehicle driver's license;

3. Some 2-inch bareheaded photos of myself;

4. Student registration form;

5. Original and photocopy of resident ID card;

6. Physical examination certificate: you need to go to the designated hospital or physical examination institution for physical examination, and provide the original and photocopy of the physical examination certificate;

7. Driving school training contract: if you obtain a driver's license through driving school training, you must provide the original and photocopy of the driving school training contract;

8. Original driver's license: If the driver's license is changed, the original driver's license and its copy shall be provided.
