The documents required to use this service include: valid passport, visa application fee of 405 yuan paid in CITIC Industrial Bank, complete visa application form, certificate of recent visit to the United States, and forms required for visas for students, visiting scholars and staff (I-20, IAP-66 or I-797 respectively). In Beijing and Shanghai, the cost of this service is 200 yuan RMB, which can be paid at CITIC Bank. Once paid, this fee will not be refunded. In Qingdao, Jinan, Zhengzhou, Nanjing, Hangzhou, Suzhou, Wuhan or Tianjin, the delivery fee is about 50 yuan RMB.
