Brief introduction of rehabilitation diseases of subacromial impingement syndrome

The subacromial space is also called supraspinatus outlet or suprahumeral space, and its upper edge is coracoid-acromial arch, which consists of the lower acromion edge, coracoacromial ligament and coracoid process. The lower margin includes the greater tubercle of humerus and the upper part of humeral head. There are rotator cuff tendon, long head tendon of biceps brachii, descending capsule of deltoid muscle and upper part of glenohumeral joint capsule in the subacromial space. In the process of upper limb elevation, the greater tubercle moves closer to the acromion, which narrows the gap, and any pathological changes in the tissue may lead to SIS. The main manifestations are shoulder pain and limited activity. Pain is usually mainly around the acromion, and sometimes it can radiate to the anchorage area of deltoid muscle, especially when the shoulder abduction is 60 ~120, the pain is the most obvious. Some patients have pain at night, and the pain is aggravated when pressing the shoulder on their side. X-ray examination can find humeral tubercle sclerosis, cystic lesions or osteophyte formation, acromion front sclerosis or osteophyte formation, subacromial space narrowing, especially at the outlet of supraspinatus muscle, which can clearly show acromion shape. The initial symptoms may be mild. Pain may radiate from the shoulder to the forearm, which may be aggravated when lifting or holding objects, and athletes may feel pain when throwing or playing tennis. When the pain continues to develop, it will produce night pain. The strength or range of motion of the upper limbs may be reduced. You may not be able to put your hands behind your back and do the action of fastening and loosening. In severe cases, the loss of mobility may lead to scapulohumeral periarthritis. In acute bursitis, there will be obvious tenderness in the shoulder. The movement of the shoulder joint in all directions is restricted and painful.
