"coccygeal pain" refers to the tail of the spine, and "coccygeal pain" refers to the pain in the coccygeal vertebra and its vicinity. There are many reasons for coccygeal pain, which we are used to divide into two categories in clinic:

First, the true coccygeal pain: the pain comes from the coccygeal pain itself, such as the pain caused by the coccygeal pain fracture.

2. Pseudococcygeal pain: Pain comes from other than coccygeal pain, such as pelvic inflammatory disease, tumor, urogenital system diseases, etc.

Anatomical structure of coccyx: coccyx usually consists of three or four small bones, and its proximal end is connected with fibrocartilage and sacrum. Generally, the range of motion of women's joints is larger than that of men, especially during pregnancy. The coccyx itself has coccygeal ligament, gluteus maximus, coccygeal muscle, anal sphincter and levator ani attached to it. In men, the coccyx is thicker than the ischium, so it has better protection when it is hit. On the contrary, in women, the coccyx is relatively easy to bruise when it falls down.

Causes of pain: The sensory nerves around the coccygeal vertebra come from the fourth and fifth sacral vertebrae and the caudal nerve itself. When the urogenital system in the pelvic cavity is diseased, the pain will involve the caudal nerve through the sacral nerve, which is the cause of pseudocaudal neuralgia.

Clinical symptoms: coccygeal pain is more common in young women. Some people have acute pain, but others are chronic diseases that can't be cured for a long time. Patients often can't sit in a normal way, and always feel more comfortable lying in bed or standing. This kind of pain is sometimes concentrated at the tip of coccyx, with tenderness locally, but some people have vague pain range. Increased stress in levator ani sometimes aggravates the pain. You should be especially careful when you cough or sneeze continuously at night, because sometimes tumors near the coccyx or spinal tumors will have similar symptoms.

Physical examination: pay attention to whether there is abnormal feeling near the coccyx, or just tenderness. Male patients must be examined for prostate abnormalities through anus. On the other hand, female patients check the pelvic genitourinary system according to symptoms. From the point of view of internal diagnosis, you can feel pain or tumor in the coccyx.

Diagnostic tool: X-ray examination of the front and side of coccyx usually shows fracture, dislocation or pseudojoint of coccyx caused by poor fracture healing. Sometimes we can also see the extent to which tumors and sacrococcygeal bones are destroyed by tumors.

Treatment: Pseudocaudal pain often comes from pelvic or urogenital diseases, so you should consult a gynecologist or urologist for further examination and treatment. Real coccygeal pain can be divided into acute pain and chronic pain. In acute pain period, non-steroidal anti-inflammatory and analgesic drugs can be given. In addition, a balloon can be given to prevent the coccyx from touching the chair when sitting, and local anesthetic plus steroid injection can achieve temporary analgesic effect. If it hurts, give it a laxative. As for patients with chronic diseases, it is recommended to take a warm bath or consider surgery. Due to the special anatomical position of coccyx, about 15% patients may be infected with gram-negative bacteria due to surgery. Because of the high complications and unsatisfactory curative effect, conservative treatment is usually adopted.
