Key points of diagnosis and treatment of cat fracture
The state of continuous partial fracture (incomplete fracture) or complete fracture (complete fracture) of bone or cartilage tissue is called fracture. Compared with dogs, cats have a lower probability of fracture. From the position, fractures such as mandible and femur often occur. Spiral fractures or oblique fractures are often seen. The process of fracture healing is relatively short. Limb fractures have walking disorders, spinal fractures have neurological disorders, mandibular fractures have eating disorders, and are often accompanied by various functional disorders.
cause
1. In the process of impact, bumping, falling, pressing or falling, when the external force exceeds the bone tension, the fracture often occurs outside its action point or action point.
2. When bone fragility, rickets or bone tumors occur, the tension of bone decreases. In this case, pathological fractures can occur under the action of external forces that usually do not lead to fractures.
symptom
1. Incomplete fractures are indistinguishable from simple impact injuries, and most cases have simple mild claudication and local swelling. When the fracture site is shallow, the periosteum swelling and pain along the fracture line can be palpated.
2. Femur, mandible and pelvis are common fractures in cats. In dogs, forearm and metacarpal fractures often occur.
3. When the limbs are completely non-open fracture, severe pain and bleeding occur at the fracture site, causing swelling, degeneration or displacement, abnormal movement, etc. And you can't carry a load when you walk. When complex fractures occur and nerves are damaged, sensory disturbance and muscle paralysis around them appear.
4. When an open fracture occurs, massive bleeding occurs due to skin shedding, but the swelling is not significant. Sometimes the broken end will be exposed.
5. When the mandible is fractured, there are symptoms such as eating and chewing disorders, salivation, mandibular pain and deformation.
6. When the skull and spine are fractured and the central nervous system is injured, symptoms such as vomiting, disturbance of consciousness or paralysis may occur according to the degree.
7. When pathological fracture occurs, primary symptoms and fracture symptoms appear at the same time.
8. In addition, due to the different degrees of fracture and surrounding tissue damage, shock or endothermic will occur. In addition, when an open fracture occurs, it will be accompanied by systemic symptoms due to bacterial infection.
9. Acute symptoms such as pain, swelling and abnormal movement after fracture are relieved or disappeared after a period of time, showing the symptoms of different healing periods of fracture. If there is no proper treatment, there will be sequelae such as deformation, rigidity, delayed healing or pseudoarthrosis.
diagnose
1. Among the complications after fracture, some symptoms need to be taken first-aid measures before fracture treatment. Therefore, in the diagnosis of fracture, we should first judge whether emergency treatment is needed through inquiry and general state examination, and then make the diagnosis of fracture after necessary treatment.
2. For open fractures or complete subcutaneous fractures, if the above symptoms appear, it is easy to diagnose. However, in order to determine the treatment plan, prognosis and dislocation or sprain, we must correctly grasp the degree of injury and pathological state. Therefore, at least the fracture site should be placed at right angles, and X-ray photography should be taken from two directions for diagnosis. Especially for incomplete fracture or occult fracture, X-ray diagnosis is necessary.
treat cordially
1. For life-threatening symptoms, such as bleeding or shock accompanied by fracture, emergency treatment is needed, such as hemostasis or anti-shock treatment.
2. For the new open fracture, after emergency treatment such as hemostasis, under general anesthesia (ketamine, 20 ~ 30 mg/kg, intramuscular injection), the local area was thoroughly disinfected to remove the damaged tissue, and then the fracture was reset and fixed to close the wound. Because bone tissue will die, rot and ossify quickly when exposed to outside air, it must be covered with soft tissue as soon as possible. In addition, most open fractures are polluted, which is very unfavorable for fracture treatment. Therefore, it is necessary to thoroughly disinfect local and systemic areas and apply enough antibiotics. Old or obviously infected open fractures, as an open treatment, are treated with local chemotherapy. Some limbs were broken and had to be amputated.
3. For cases with early simple subcutaneous fractures that can be fixed without bleeding, general anesthesia should be used as soon as possible, and traction should be carried out along the long axis of the affected limb, so that the fracture end occlusion can be restored to the normal position through lateral pressure or flexion. On this basis, it is firmly fixed with plastic or splint. Because fixation is directly related to the prognosis, in principle, not only the local part of the fracture, but also the joints at both ends of the fracture should be fixed. In addition, due to early swelling of the affected area, bandages are often overstressed. Therefore, it is usually necessary to pay attention to the state of the remote part.
4. For complex or non-hemorrhagic fractures that are difficult to recover over 1 week, open reduction and fixation are needed. Cut the skin under general anesthesia, take out the soft tissue wrapped in the fracture under direct vision, and then restore it to its normal position and fix it. As a fixing method, the inner shaft of the metal bar can be fixed and stainless steel wires can be connected. Generally, after suturing the skin, external fixation is adopted.
5. For the mandibular fractures that often occur in cats, stainless steel wire connective tissue is generally used for fixation.
6. For the old fracture that has been deformed and healed, it can also be cut and then osteotomy and then reset. In addition, some cases of delayed treatment or pseudoarthrosis are suitable for bone transplantation.
7. The prognosis of cat fracture is generally good. For pure fracture, it can heal in about 4 ~ 6 weeks after non-blood external fixation.
