Lost application for replacement of tour guide card

Please reissue the tour guide card.

If the holder loses his certificate, he should report the loss to the unit and reissue it in time, and go to the issuing authority with the certificate of loss, ID card (and its copy), tour guide qualification certificate and its copy issued by the unit to go through the formalities of replacing the lost certificate: fill in the Tour Guide Registration Form (in duplicate) and mark "Lost Replacement". On the basis of this, the holder applies for the tour guide registration form, and contacts China Tourism News and Provincial Daily to publish the Declaration on Cancellation of Report of Loss of Tour Guides (including the number of tour guides, names and license numbers). After 1 month from the date when the tour guide's report of loss is invalid, the tour guide shall submit the report of loss to the original issuing authority with the notice of newspaper publication, tour guide's qualification certificate, ID card and the certificate issued by the unit where he works.
