Physical examination items:

1. Note:

1. You do not need to fill in the physical examination form yourself. The hospital staff will fill it out for you during the physical examination.

2. If you do not have a 2-inch ordinary frontal color photo during the physical examination, you can use a passport photo or a 50-50cm photo. It never hurts to bring more photos.
