What materials do I need to apply for a tourist visa to Slovakia?

1. The visa application form must be completed completely, clearly, truly and correctly in capital Roman letters and signed by myself. The application form shall be submitted by the applicant himself (if the applicant is under 15, it shall be signed by his legal representative).

2. A recent passport photo (posted at the designated position in the application form).

3. A valid passport. Passports are valid for at least 90 days longer than their stay in Sri Lanka.
