1, symptoms of gastroesophageal reflux disease
Gastroesophageal reflux disease is a gastrointestinal motility disorder caused by many factors, which is the result of the decline of anti-reflux mechanism and the influence of reflux on esophageal mucosa. Gastroesophageal reflux disease (GERD) has different clinical manifestations due to different course and severity, and is influenced by many factors. It may have the following symptoms:
1 burning sensation behind sternum:
The burning sensation behind the sternum, also known as reflux heartburn, is the main symptom of this disease, which often occurs after meals 1 hour or so, and is often related to posture, such as bending, bending, coughing, pregnancy, ascites, forced defecation and other postures to increase the pressure in the stomach, which can induce aggravated heartburn, also known as postural heartburn. It can also be induced by overeating and taking strong tea, wine, coffee, juice and aspirin. The above symptoms are typical in patients with esophageal hiatal hernia, accompanied by abdominal distension, burping, belching and burning sensation of oral mucosa. After taking antacid drugs, the symptoms can disappear temporarily, and eating overheated or acidic stimulating food can aggravate the condition. If the effect of taking antacids is not obvious, it suggests that gastric acid deficiency and burning sensation are mainly caused by bile reflux. If severe esophagitis is scarred, there may be no or only a slight burning sensation. Traditional Chinese medicine believes that the pathogenesis of the disease is due to weakness of the spleen and stomach, excessive yin fire, disharmony of the stomach, and rising yin fire with turbid qi in the stomach. These fires, which are separated from the meridians, are all in the middle of the spleen from the middle jiao to the upper jiao, so the clinical manifestations are the burning sensation behind the sternum (or under the sternum).
2 pain behind the sternum or upper abdomen:
The pain of this disease is similar to angina pectoris or pleurisy, which radiates to the back, chest and even behind the ears. The severe pain is a severe stabbing pain. If patients with reflux esophagitis have persistent retrosternal pain, or even radiate to the neck, it is suggested that it is a penetrating marginal ulcer or esophagitis. Paroxysmal cough, wheezing and even suffocation may occur at night when the intercepted food and reflux secretions are inhaled into trachea and lungs. The main causes are emotional internal injuries, improper diet or overwork. Stagnation of liver qi, qi disorder, qi stagnation, pain on both sides of the spleen and stomach and behind the sternum, often accompanied by hypochondriac pain, frequent belching, which is caused by discomfort. Qi stagnation easily leads to blood stasis, so the pain is more severe.
3 Reflux to throat:
The patient described that acid liquid food enters the stomach and esophageal reflux to the mouth and pharynx every time the patient takes the front position or lies flat after a meal, which mostly occurs before or after the burning sensation under the sternum. The lesion of this disease is located at the intersection of upper and middle jiao, which belongs to the part of ren meridian. Stomach qi declines, but spleen and stomach weakness leads to liver depression, so the stomach qi does not decline, and the meridian qi of Ren meridian also flows to the upper diaphragm of the middle and upper abdomen to Li Anquan's pharynx. Therefore, the turbid gas in the stomach of patients with reflux esophagitis combines with yin fire and flows back to the pharynx, resulting in reflux chronic pharyngitis.
4. Adverse swallowing or vomiting:
They interact with each other, making it difficult to swallow, eating unfavorably, and vomiting against the upward trend. In the early stage of the disease, due to inflammation, esophageal spasm is limited, which can cause temporary dysphagia, dysphagia and vomiting. With the development of the disease, persistent dysphagia and vomiting may occur in the later stage due to stenosis caused by fibrous scar.
When dysphagia becomes more and more serious, the frequency and degree of heartburn gradually decrease, and even there is no heartburn in the later stage.
Five special types of esophagitis:
When you see suppurative esophagitis in clinic, it often manifests as pain behind the sternum and neck, or dysphagia and pain. If the lesion is extensive, fever, chills and general malaise may occur. Candidal esophagitis can also be seen clinically. This type of symptoms are only dysphagia and mild swallowing pain. Severe cases may present severe pain behind the sternum, even fear of eating, weight loss, anorexia, vomiting and even bleeding.
Gastroesophageal reflux disease is closely related to people's usual diet structure and living habits. The best way to keep healthy is to nip in the bud and cultivate good living habits. Remind patients: reduce the amount of food per meal, eat less fried, fried and high-fat foods, ban smoking and alcohol, and try to eat less food that can cause reflux, such as pepper, coffee, mint and chocolate.
2. Etiology of gastroesophageal reflux disease.

Zheng Naiyuan analyzed that the increase of gastroesophageal reflux in modern people is related to living habits and diet. Many people like to eat midnight snack, but when they eat too close to bedtime, their stomachs are full of food, which stimulates the secretion of gastric acid. When the stomach is enlarged, the cardia of the esophagus will also be enlarged by controlling the food not to reflux. After eating, I go to bed. When I lie on my back, things in my stomach tend to flow backwards because of the potential difference caused by insufficient gravity.
He said that many people like to drink coffee and strong tea, but coffee and strong tea will also relax the tension in the cardia and increase the secretion of gastric acid.
Zheng Naiyuan said that from the results of physical examination, men, middle-aged people, smoking and drinking, and obese people are the four major risk groups. Men are more prone to gastroesophageal reflux than women, because men generally don't pay much attention to their living and eating habits, socialize more, and smoke more. Obese people tend to overeat and produce negative pressure, which makes it easier to go upstream.
Zheng Naiyuan reminded that the most important thing to prevent or improve reflux esophagitis is to adjust daily diet and lifestyle, quit smoking and drinking, reduce coffee and strong tea, and maintain normal weight; The order of eating should be based on the principle of separation of dry and wet, with soup before solid food, moderate diet, less irritating food, only 7 minutes full for each meal, and no eating for 3 to 4 hours before going to bed.
