The third line: gender, date of birth

Please tick the box corresponding to gender.

Date of birth: Fill in the numbers in chronological order. For example, if you were born in September 1996, 1, then fill in 0 19/ 1996.

Line 4: Town and country of birth

Write Chinese Pinyin or corresponding English names, such as Beijing and Hongkong, and fill in China for the country.

The fifth line: nationality and occupation

Fill in China internationally.

Line 6: Contact address in the UK.

This can fill in the full and detailed address of your dormitory, hotel or relatives. Generally, it is filled in the order of room number, hotel address name, street name, city or town name and postal code.
