1. Voluntary participation in Suqian Infinite Pole Parkour Commune, hereby declare and guarantee that:

I am 18 years old with full capacity. I voluntarily participate in the activity and make this statement and guarantee based on my true wishes.

I am the guardian of a person with incomplete capacity below 18, and I make this statement and guarantee on behalf of the true will of the ward.

2. Health statement:

Before I took part in parkour, I was told that patients with the following diseases were not suitable for this activity. I am healthy and can participate in community activities without any of the following diseases.

(1) Congenital heart disease and rheumatic heart disease; (2) Hypertension and cerebrovascular diseases; (3) heart diseases such as myocarditis; (4) Patients with coronary heart disease and severe arrhythmia; (5) Diabetic patients with hyperglycemia or hypoglycemia; (6) Other diseases unsuitable for exercise.

3. Safety statement:

(1) I know that parkour, as an extreme sport, has certain known and/or unpredictable security risks, which may cause personal and property losses to me and/or a third party.

These risks include but are not limited to:

A. Fall and collide during exercise; B. one's own physical condition, accompanied by the physical reaction of the exercise itself; C. damage or loss of personal equipment and articles, etc.

(2) As a person with full capacity (guardian of a person with incomplete capacity), I have a prudent obligation to protect my (ward's) person and property from various known and/or unforeseeable security risks mentioned in Article 3 of this statement (1).

(3) I hereby make it clear that if I (the ward) have any safety accidents in parkour activities, I will bear all corresponding consequences and responsibilities, and the organizers and managers of parkour activities need not bear any economic and/or legal responsibilities for any safety accidents.

(4) Suqian Infinitus Parkour Commune has forced all participants to buy accident insurance. I shall purchase accident insurance in time according to the requirements of Suqian Infinitus Parkour Club, and guarantee that the accident insurance is valid for 2 calendar days including the date of registration. If you don't buy it, all safety accidents will happen at your own risk!

(5) I know that taking out accident insurance cannot exempt me from the obligation of careful protection for my personal and property safety, nor will it cause the organizers and managers of parkour activities to bear any economic and/or legal responsibilities for my personal and property safety.

4. Public safety statement:

I understand that in parkour activities, no team member is allowed to bring the following items to participate in the activities:

A. Firearms and/or controlled knives; B. toxic substances; C. Flammable and explosive articles; D. lighting matches; E. other items that may endanger public safety. If I take the above items to participate in parkour activities, the economic and/or legal responsibilities arising therefrom shall be borne by me.

5. obey orders:

(1) I know the rules of parkour activities and obey the command and arrangement of the management personnel of Suqian Infinitus Parkour Commune; (2) If I violate the command of the management personnel of Suqian Infinitus Parkour Commune and engage in personal activities unrelated to parkour activities, all the consequences arising therefrom shall be borne by me.

6. authorize the use of audio-visual products.

I know that any photo, film, video tape, audio tape or other audio-visual products obtained by shooting, recording or recording images and/or sounds in related activities are used in any promotional materials (including but not limited to the official website) of parkour activities or in any promotion or public welfare activities of the activities without consulting anyone or obtaining anyone's consent in advance, and without paying anyone any fees for it.
