What is the diagnostic value of CR for breast cancer? Seek an answer

With the increasing incidence of breast cancer, molybdenum target photography has become one of the important methods for clinical diagnosis of breast cancer. However, ordinary mammography can not clearly show the contrast between the fine structure and glands in the soft tissue of the breast. With the appearance and wide application of computerized X-ray photography (CR), the imaging methods of breast tissue structure have been greatly improved, which is helpful for the diagnosis of breast cancer.

In this group 120 breast cases were diagnosed as malignant lesions by CR, all of which were confirmed by postoperative pathology. The report reads as follows:

1. Data and methods

All the patients in this group 120 were female, with the onset age ranging from 28 to 74 years, with an average of 43.8 years. Most of them are palpable breast masses, most of them have no conscious symptoms, and a few are accompanied by swelling pain. Physical examination: the lump is hard, the surface is not smooth, the edge is irregular, the boundary with surrounding tissues is unclear, and the mobility is poor, so it is not easy to be pushed. Some nipples are filled with exudate, which invades local skin. Most axillary lymph nodes are swollen, and a few supraclavicular lymph nodes are swollen. There were 56 cases of left breast and 64 cases of right breast.

GIOTTO600 1 mammography camera takes pictures of head-tail position (CC) and medial-lateral oblique position (MLO) on both sides of the breast, and GE Healthcare Performa MGF-110 equipment takes pictures of head-tail position (CC) and medial-lateral oblique position (MLO)CR films on both sides of the breast. Among the 120 breast cancer patients examined, 5 cases were advanced breast cancer, 22 cases were diagnosed as false negative by X-ray taken by GIOTTO600 1 mammography camera, and 20 cases were diagnosed as false positive. Observation after treatment with CR technology: ① The diagnosis of early breast cancer (including breast cancer to the highest grade) is further clarified. ② Identify the false positive and false negative of common molybdenum target X-ray films. ③ To determine the clinical value of CR in the clinical diagnosis of breast cancer by comparing the image diagnosis of CR film with postoperative pathological results.

2. Results (1) CR film can present images with high density, high definition and rich layers.

Using the harmonic processing technology in CR technology, the window width and window level of the image are changed, and the contrast of the image is adjusted, so that it is easier and more accurate to read CR films in mammography. CR can also perform local enlargement, size measurement and black-and-white inversion of the lesion, which improves the ability of displaying the lesion and the accuracy of diagnosis.

(2) High-quality CR images increase the information of pathological changes.

Mammary molybdenum palladium CR film is a high-definition digital mammogram, which is recorded by carrier IP board, read by laser and processed by image. In this X-ray film, the image density of normal tissue and pathological tissue is obviously different, which clearly presents diagnostic images, such as local dense infiltration and microcalcification, tumor edge burr sign and other unique X-ray signs of breast cancer.

(3) The accuracy of breast cancer diagnosis is improved.

120 cases of breast cancer Compared with ordinary X-ray films, CR films can obviously improve the accuracy of clinical diagnosis and pathological diagnosis of breast cancer. The diagnostic coincidence rate of ordinary X-ray film was 83.5%, and that of CR film was improved to 95.8%. The diagnostic coincidence rate of common X-ray film and CR film in breast cancer group was 63% and 90%, respectively. Among the 20 patients who were initially diagnosed as positive, 65,438+06 cases were excluded, of which 6 cases were suspected as malignant lesions by ordinary X-ray. After CR treatment, it is observed that the internal changes of the original shadow are uniform and equidensity. Among the initial 22 false negative patients, 15 cases were definitely diagnosed.

Breast cancer is one of the common malignant tumors in women. In recent years, the incidence rate is on the rise, and there is a trend of youthfulness. Mammography is of great significance for early detection and diagnosis of breast cancer. Ordinary mammography can't adapt to the development of modern medicine. Examination of breast malignant lesions, such as mass shadow and early microcalcification, is easy to be missed and misdiagnosed, and cannot clearly and accurately reflect the real situation. With the wide application of CR technology, the generation of digital images has largely solved this problem and improved the detection rate of early breast cancer.

3. Discussion (1) Since 1969 Gro, France first used molybdenum target X-ray tube for mammography, the diagnostic efficiency of breast diseases has been greatly improved. Mammography is the most important and effective method to diagnose breast diseases, especially to find early breast cancer, and it is also the first choice to diagnose breast cancer stages. Mammary molybdenum target CR film is clear and easy to read, with good contrast. Post-processing technology can change the contrast of the image through coordinated processing, realize the adjustment of contrast and optical density, and thus achieve the best display effect. In the traditional intensifying screen/film X-ray photography system, if the appropriate exposure dose of molybdenum target X-ray is given, a good mammogram can be obtained. If the exposure dose is too high or too low, the obtained image cannot be diagnosed effectively. CR system can adjust the read image to meet the diagnostic requirements by using the unique exposure latitude of IP board, thus providing higher and more valuable diagnostic basis for clinic.

(2) Conventional mammography is difficult to accurately display the tissue structure and lesions. CR mammography can find subclinical breast lesions that cannot be palpated, especially small nodular breast cancer and calcified breast cancer, which are often difficult to display on ordinary mammography. In addition, on X-ray film of molybdenum target, it is difficult to distinguish lump type with tiny sediment-like calcification, small nodule type with sediment-like calcification, actual deformation of normal breast, small nodule type lesion and surrounding dense ductal shadow or hyperplasia shadow, calcification, fibrosis, needle-like change of lobulation edge and edge, and low-density shadow around cancer, but CR image can show the above signs, and it is also difficult to distinguish comet sign, tubular infiltration, miliary lump and positive ". CR molybdenum target photography is superior to ordinary molybdenum target photography in resolution, clarity and fine structure display of lesions. It has obvious advantages in improving the diagnosis rate of early breast cancer and reducing the false positive rate and false negative rate, and has wide clinical application value.
