Do you need a physical examination form when applying for a visa?
Currently no physical examination form is required.
Take the European Schengen visa application as an example:
1. Passport valid for more than six months;
2. Copies of household registration and ID card;
Certificate of deposits above RMB 33,000 frozen for three months;
4. Copy of the unit’s business license or organization code certificate;
5. Proof of employment;
The above are a few basic materials. You can also provide valid subsidy materials such as real estate certificates or car purchase invoices.
If the material is not very good, a deposit may be involved.
The materials required for a Schengen visa are relatively complete
If it is a visa from Southeast Asia or Africa, the materials are very simple. Only a passport, photo and information form are needed
