Criteria for judging hidden dangers of major accidents in coal mines

Article 1 In order to accurately identify and promptly eliminate the hidden dangers of major accidents in coal mines, this standard is formulated in accordance with the Law of People's Republic of China (PRC) on Safety in Production, the Special Provisions of the State Council on the Prevention of Coal Mine Safety Accidents (Order No.446th of the State Council) and other laws and administrative regulations.

Article 2 This standard is applicable to the determination of hidden dangers of major accidents in various coal mines.

Article 3 The hidden dangers of major accidents in coal mines include the following aspects:

(a) super capacity, super strength, super quota organization of production;

(2) gas overrun operation;

(three) coal and gas outburst mines, not in accordance with the provisions of the implementation of outburst prevention measures;

(four) the gas drainage system and monitoring system have not been established in the high gas mine, or the system can not operate normally;

(5) The ventilation system is imperfect and unreliable;

(six) a serious flood occurred and no effective measures were taken;

(seven) super layer cross-border mining;

(eight) there is a danger of rock burst, and no effective measures have been taken;

(9) Serious spontaneous combustion and failure to take effective measures;

(ten) the use of prohibited or eliminated equipment and technology;

(eleven) there is no double circuit power supply system in coal mine;

(12) When a new coal mine is built, the regional production or other regional production exceeds the scope and scale specified in the design of safety facilities when the coal mine is rebuilt and expanded;

(thirteen) after the overall contract production and operation of coal mine, it fails to re-obtain or change the safety production license in time to engage in production, or the contractor subcontracts again to contract the maintenance work of underground mining face and roadway with labor services;

(fourteen) the person in charge of production safety and the safety management organization were not clearly defined during the coal mine restructuring, or the mining license, safety production license and business license were not re-acquired or changed after the restructuring;

(15) Other major accidents.

Article 4 The hidden danger of "organizing production with super capacity, super strength and super quota" refers to any of the following circumstances:

(1) The annual output of raw coal in coal mines exceeds the approved (designed) production capacity 10% or the monthly output of raw coal exceeds the approved (designed) production capacity 10% or more;

(two) the coal mine or its superior company exceeds the approved (designed) production capacity of the coal mine and issues production plans or business indicators;

(3) The recoverable period of coal mining, preparation and recovery is less than the shortest time stipulated by the state, and production is still organized without actively limiting or stopping production (except for aging coal mines and local people's governments that plan to stop production and close coal mines);

(4) The coal mine has more than two production levels at the same time, or the number of coal mining and coal roadway (semi-coal rock roadway) heading faces operating at the same time in a mining area (panel) exceeds the provisions of the Coal Mine Safety Regulations;

(five) the gas drainage is not up to the standard to organize production;

(six) the coal mine has not formulated or strictly implemented the underground labor capacity system, or the number of single shifts at the mining operation site exceeds the relevant provisions of the state by more than 20%.

Article 5 A major accident of "gas over-limit operation" refers to any of the following circumstances:

(a) there are cases of missing or false detection in gas inspection and operation;

(two) continue to work after the underground gas exceeds the limit or continue to work without disposal in accordance with state regulations;

(three) failing to formulate and implement safety technical measures to discharge gas accumulated underground in accordance with state regulations.

Sixth "coal and gas outburst mine, not in accordance with the provisions of the implementation of outburst prevention measures" of major accidents, refers to one of the following circumstances:

(a) without the establishment of outburst prevention institutions and equipped with corresponding professionals;

(two) the ground permanent gas drainage system has not been established or the system can not operate normally;

(3) Failing to predict the outburst danger of an area or working face in accordance with state regulations (except those directly identified as outburst danger areas or outburst danger working faces);

(four) failing to take outstanding prevention and control measures in accordance with state regulations;

(5) failing to inspect and identify the effect of outburst prevention measures in accordance with state regulations, or organizing production and construction if the inspection and identification of outburst prevention measures are unqualified, or forging inspection and identification data of outburst prevention measures;

(six) failing to take safety protection measures in accordance with state regulations;

(seven) the use of overhead electric locomotives.

Seventh "gas extraction system and monitoring system have not been established in high gas mine, or the system can not operate normally" refers to one of the following circumstances:

(a) in accordance with the provisions of the "coal mine safety regulations", the gas drainage system should be established but not established or the system is not in normal use;

(2) failing to install or adjust the methane sensor according to the provisions of the state, resulting in the artificial failure of the methane sensor, or failing to give an alarm or cut off power after the gas exceeds the limit, or failing to meet the provisions of the state.

Article 8 "Imperfect and unreliable ventilation system" refers to one of the following situations:

(a) the total air volume of the mine is insufficient or the air volume of the main wind sites such as the mining face is insufficient;

(two) there is no spare main ventilator, or two main ventilators do not have the same ability;

(three) in violation of the provisions of the "coal mine safety regulations" series ventilation;

(four) the ventilation system is not formed according to the design, or the production level and the mining area (panel) are not ventilated by districts;

(5) In any mining area (panel) of the mine with high gas, coal and gas outburst, the coal seam with easy spontaneous combustion is mined, and the coal seam with low gas is mined, and the mining area (panel) with joint layout is adopted for layered mining, and there is no special return air lane, or there is no independent return air system in the working face of the outburst coal seam;

(6) The wind wall and damper of the communication lane between the inlet and return air shafts and between the main inlet and return air lanes do not conform to the provisions of the "Coal Mine Safety Regulations", resulting in short circuit of airflow;

(7) The intake and return air lanes in the mining area do not run through the whole mining area, or a section of intake air and a section of return air run through the whole mining area, or inclined long walls are arranged, and the alleys do not advance by at least two sections to form a ventilation system, that is, other lanes are excavated;

(eight) the coal roadway, semi-coal roadway and rock roadway with gas emission are not equipped with methane electricity and wind power locking devices or related devices in accordance with state regulations;

(9) When local ventilation is used in coal roadway, semi-coal roadway and rock roadway with gas emission in high gas, coal (rock) and gas (carbon dioxide) outburst mines, double fans and dual power sources cannot be automatically switched;

(ten) high gas, coal (rock) and gas (carbon dioxide) outburst construction mines before entering the second phase of the project, other construction mines before entering the third phase of the project, did not form a full wind pressure ventilation system for the main fans on the ground.

Ninth "serious flooding, no effective measures" of major accidents, refers to one of the following circumstances:

(a) did not find out the mine hydrogeological conditions within the scope of large mined-out areas, abandoned old kiln water and other conditions and organize production and construction;

(two) the mine with complicated hydrogeological types and extremely complicated mine is not equipped with special water prevention and control institutions, special water exploration and drainage operation teams and special water exploration and drainage equipment;

(three) mining in areas that need to be explored and drained, and failing to conduct exploration and drainage in accordance with state regulations;

(four) failing to set up or mine (destroy) various waterproof coal (rock) columns in accordance with state regulations;

(five) there are signs of sudden (seepage, collapse) water has not been completely evacuated from the underground personnel threatened by floods;

(six) the mine threatened by surface water backflow failed to stop production and evacuate personnel during the period of heavy rainfall or flood in the upstream of its incoming water;

(seven) before the mine construction entered the third phase of the project, the permanent drainage system was not built according to the design, or when the production mine was extended to the design level, the waterproof and drainage system was not built and the excavation was illegal;

(eight) the drainage capacity and water storage capacity of the pumps and pipelines of the main drainage system of the mine do not meet the requirements of the "coal mine safety regulations";

(nine) mining surface water, goaf water area or steep coal seam under strong aquifer, and failing to eliminate the threat of flood in accordance with state regulations.

Tenth "super-layer cross-border mining" major accident refers to one of the following circumstances:

(a) mining beyond the level or elevation of the seam specified in the mining license;

(2) Mining beyond the coordinate control range specified in the mining license;

(three) unauthorized mining (destruction) of safe coal pillars.

Eleventh "there is a risk of rock burst, and no effective measures have been taken" refers to one of the following circumstances:

(1) Failing to identify the impact tendency of coal seams (rock strata) in accordance with state regulations, or failing to evaluate the impact risk of coal seams with impact tendency, or failing to evaluate the impact risk of mining areas and mining faces when mining coal seams with impact ground pressure;

(two) the mine with the risk of rock burst has not set up a special anti-scour mechanism, is not equipped with professionals or has not prepared a special design;

(three) the risk of rockburst is not predicted, or the effect of anti-scour measures is not tested, and the effect of anti-scour measures is not up to standard, but the production and construction are still organized;

(4) Mining isolated coal pillars in violation of regulations when mining rockburst coal seams, the position and spacing of mining faces do not conform to the provisions of the state, or the mining sequence is unreasonable, the mining speed does not conform to the provisions of the state, the roadway is arranged in violation of the provisions of the state, or coal (rock) pillars are left, resulting in stress concentration;

(five) the personnel access system in the dangerous area of rock burst has not been formulated or strictly implemented.

Twelfth "serious spontaneous combustion, no effective measures" of major accidents, refers to one of the following circumstances:

(a) the mining of mines prone to spontaneous combustion and spontaneous combustion of coal seams, without the preparation of special fire prevention and extinguishing design or without taking comprehensive fire prevention and extinguishing measures;

(two) the use of top coal caving mining method in high gas mines can not effectively prevent spontaneous combustion of coal seams;

(three) there are signs of spontaneous combustion and no corresponding safety measures have been taken to continue production and construction;

(four) in violation of the provisions of the "coal mine safety regulations" unsealed fire area.

Thirteenth "the use of prohibited or eliminated equipment and technology" major accidents, refers to one of the following circumstances:

(a) the use of products or processes listed in the catalogue of equipment and processes prohibited by the state from being used underground in coal mines;

(two) the underground electrical equipment and cables have not obtained the safety signs of coal mine products;

(3) the selection of underground electrical equipment does not meet the mine gas level, or the explosion-proof electrical equipment in the mining area (panel) is lost, or the non-explosion-proof trackless rubber-tyred vehicle is used underground;

(4) Failing to select the corresponding permissible explosives and detonators for coal mines according to the gas grade of the mine, not using special blasting equipment, or carrying out bare blasting;

(five) the coal mining face can not guarantee the smooth flow of two safety exits;

(six) high gas mines, coal and gas outburst mines, spontaneous combustion and spontaneous combustion coal seam mines (except thin coal seam), the coal mining face adopts forward mining method.

Fourteenth "coal mine unparalleled loop power supply system" major accident hidden danger refers to one of the following circumstances:

(a) single-loop power supply;

(two) there are double-loop power supply lines, but they are taken from the same bus section of the regional substation;

(3) The high gas, coal and gas outburst, complicated and extremely complicated construction mines entering the second phase of the project, and other construction mines entering the third phase of the project did not form a double-circuit power supply.

Fifteenth "new coal mine construction and production, coal mine reconstruction and expansion period, production in the reconstruction and expansion area, or production in other areas outside the design scope and scale of safety facilities" refers to one of the following circumstances:

(a) the design of safety facilities of construction projects has not been examined and approved, or major changes have been made after examination and approval, and the construction has been organized without re-examination and approval;

(two) the new coal mines organize coal mining during the construction period (except for the approved joint trial operation);

(three) the reconstruction and expansion of the mine is produced in the reconstruction and expansion area;

(four) the mine in the non-reconstruction and expansion area is expanded beyond the design scope and production scale.

Article 16 "A major accident hidden danger refers to any of the following circumstances: after the overall contracted production and operation, the coal mine fails to re-obtain or change the safety production license in time to engage in production, or the contractor subcontracts again to contract the maintenance work of the underground mining face and roadway with labor services":

(a) the coal mine is not contracted in the form of overall contract, or the whole coal mine is contracted to a unit or individual that does not have legal person status or has not obtained a legal and valid business license;

(two) the implementation of the overall contract of coal mines, the safety production management agreement has not been signed, or the safety production management responsibilities of both parties have not been agreed in accordance with state regulations;

(three) the implementation of the overall contract of coal mines, without re obtaining or changing the safety production license for production;

(four) the implementation of the overall contract of coal mines, and the contractor subcontracts the coal mines to other units or individuals again;

(5) contracting out the underground mining operation or roadway maintenance operation (except the new horizontal extension of the bottom of the well, the main transportation, main ventilation, main drainage and the development project of the main electric chamber) to other enterprises or individuals as independent projects, and subcontracting the development project of the new horizontal extension of the underground.

Article 17 "The person in charge of safety production and the safety management organization are not clearly defined during the coal mine restructuring, or the mining license, safety production license and business license are not re-acquired or changed after the restructuring" refers to any of the following circumstances:

(a) during the restructuring period, the person responsible for production safety was not clearly defined for production and construction;

(two) during the restructuring period, the organization of safety production management was not improved, and safety management personnel were equipped for production and construction;

(three) after the completion of the restructuring, the production and construction are carried out without re-obtaining or changing the mining license, safety production license and business license.

Eighteenth "other major accidents" refers to any of the following circumstances:

(a) not equipped with full-time mine manager, chief engineer and deputy mine manager in charge of safety, production and electromechanical, and professional and technical personnel in charge of coal mining, tunneling, electromechanical transportation, ventilation, geological survey and water prevention;

(2) Failing to fully withdraw or use the safety production expenses in accordance with the provisions of the state;

(3) Failing to carry out gas grade appraisal in accordance with state regulations, or practicing fraud in gas grade appraisal;

(4) Gas dynamic phenomenon occurs, or an outburst accident occurs in the same coal seam mined by adjacent mines, or it is identified as an outburst coal seam. Non-outburst mines whose coal seam gas pressure reaches or exceeds 0.74MPa fail to immediately identify the outburst risk according to the management of outburst coal seam within the time limit prescribed by the state (except those directly identified as outburst mines);

(5) When the mine manager or chief engineer (technical director) implements the post responsibility system and management system for production safety, he forges drawings, conceals the mining face, provides false information, conceals the number of people going into the well, or falsifies records;

(six) the mine safety monitoring system, the personnel position monitoring system or the system can not operate normally, and the system data is modified, deleted or shielded, or there are coal and gas outburst mines listed in the second paragraph of Article 7;

(seven) the hoist for lifting (transporting) personnel is not equipped with protective devices in accordance with the provisions of the "Coal Mine Safety Regulations", or the protective devices are invalid or overloaded;

(8) The conveyor belt of the belt conveyor failed to pass the third-party flame retardant and antistatic performance test before entering the well, or the test was unqualified, or the protective devices such as anti-skid, deviation and coal piling of the conveyor belt or the temperature and smoke monitoring devices failed;

(9) When the roadway behind the heading face or the single-headed roadway is maintained (fire point and high temperature point treatment), the maintenance (treatment) point is used to continue heading or personnel enter, or the heading face is not equipped with compressed air, water supply and communication lines and devices according to the provisions of the state;

(ten) the slope angle of open-pit coal mine is greater than the design maximum, or the slope deformation is serious and measures are not taken in time;

(eleven) other major accidents identified by the national mine safety supervision institutions.

Article 19 The national regulations mentioned in this standard refer to relevant laws, administrative regulations, departmental rules, national standards and industry standards, as well as administrative normative documents formulated by the State Council, its emergency management departments and the national mine safety supervision institutions according to law.

Article 20 This standard shall come into force as of 20211. The former State Administration of Work Safety (2065438+200565438+February 3, 2005) published the Criteria for Determining the Hidden Dangers of Major Production Safety Accidents in Coal Mines (Order No.85 of the State Administration of Work Safety) shall be abolished at the same time.
