Local causes (1) Special infection: Candida vaginitis and trichomonal vaginitis are the most common causes of vulvar itching. Crabs and scabies can also cause itching. Itching around anus and vulva caused by pinworm usually occurs at night. (2) Chronic vulvar malnutrition: pruritus is the main symptom, accompanied by vulvar skin whitening. (3) Drug allergy or chemical stimulation: soap, condoms, bromogeramine, etc. Contact or allergic dermatitis may be caused by direct stimulation or allergy, and symptoms of vulvar itching may occur. (4) Bad hygiene habits: If you don't pay attention to the local cleaning of the vulva, sebum, sweat, menstrual blood, vaginal secretions, and even urine and feces are soaked, and long-term stimulation of the vulva can cause itching; Using unclean menstrual belt during menstruation and wearing airtight chemical fiber underwear at ordinary times will induce itching due to damp-heat stagnation. (5) Other skin lesions: abrasions, common warts, herpes, eczema and tumors can all cause vulvar itching. 2. Systemic diabetes (1): Due to the stimulation of diabetes to vulva skin, especially when accompanied by candidal vulvitis, vulva itching is serious. Many patients first went to the hospital because of pruritus and redness of vulva, and were diagnosed as diabetes after further examination. (2) Vulvar pruritus in chronic patients such as jaundice, vitamin A and vitamin B deficiency, anemia and leukemia is often a part of systemic pruritus. (3) Intrahepatic cholestasis of pregnancy can also cause itching all over the body, including vulva. (4) Congestion of vulva during pregnancy and premenstrual period may occasionally lead to pruritus and discomfort of vulva. (5) Unexplained vulvar pruritus: Some patients have severe vulvar pruritus, but no obvious systemic or local causes can be found. At present, some scholars think it may be related to mental or psychological factors. The clinical manifestations and diagnosis of vulvar pruritus are mostly located in clitoris and labia minora, and can also spread to labia majora, perineum and even perianal lesions, which are often paroxysmal or persistent and worsen at night. The degree of itching varies significantly with diseases and individuals. Long-term scratching can cause skin hypertrophy and lichenoid changes. The itching caused by crabs is mostly confined to pubic hair and its vicinity, and scratching can cause scratches, blood scabs or secondary folliculitis. Look carefully for gray crabs about 1 ~ 2 mm long scattered between pubic hair. Unexplained vulvar pruritus generally only occurs in women of childbearing age or postmenopausal women, involving the whole vulva, but it can also be limited to a certain part or unilateral vulva. Although itching is very serious and even unbearable, the appearance of local skin and mucosa is normal, or there are only scratches and blood scabs caused by excessive scratching. When making a diagnosis, we should inquire about the course of disease in detail, carefully carry out local and general examinations and necessary laboratory examinations, and try our best to find out the cause. Treatment of vulvar pruritus 1. General treatment should pay attention to menstrual hygiene, keep vulva clean and dry, and avoid scratching. Don't wash and perm with hot water, and avoid using soap. When there is infection, potassium permanganate solution can be used for sitz bath, but local scrubbing is strictly prohibited. Clothes, especially underwear, should be loose and breathable. Avoid alcohol and spicy or allergic food. 2. Etiological treatment eliminates local or systemic factors that cause itching, such as trichomonas, candida infection or diabetes. If you find crabs, shave your pubic hair and wash your underwear, or use L% malathion powder or 25%-50% Stemona tincture for external use. 3. Symptomatic treatment ① External use: For acute inflammation, you can apply 1% Rezocine plus L- Zhenlifano solution or 3% hydrochloric acid solution, and then apply 40% zinc oxide ointment locally after washing; Chronic itching can be wiped with corticosteroid ointment or 2% diphenhydramine ointment. ② Oral drugs: When the symptoms are serious, chlorpheniramine 4mg, diphenhydramine 25rug or promethazine 25mg should be taken orally for sedation and desensitization.
