What are the medical examination hospitals designated by American visas?

1 Beijing International (SOS) Rescue Center, Xingfu Sancun North Street, Chaoyang District, Beijing 1;

Beijing International Medical Center-Beijing Tongren Hospital, No.50 Liangmaqiao Road, Beijing;

Beijing United Family Jianguomen Health Care Center, No.21Jianguomenwai Street, Chaoyang District, Beijing;

Fuzhou East Street 134 Fujian Provincial Hospital;

Guangdong International Travel Health Care Center, No.33 Shamian North Street, Guangzhou;

International Health Center of Shanghai First People's Hospital, No.585 Jiulong Road, Hongkou District, Shanghai;

Taishan People's Hospital, No.80 Huanbei Road, Taishan City, Guangdong Province.

Physical examination mainly excludes the following diseases:

Cholera, syphilis, yellow fever, active tuberculosis, plague, mental retardation, leprosy, mental illness.

You must bring:

1, passport

2. Letter of appointment.

3. Physical examination form (DS-2053, DS-3024, DS-3025, DS-3026), ***5 sheets of paper.

4. 4 ordinary 2-inch front color photos (4 for children).

note:

1. You don't need to fill in the physical examination form yourself. The hospital staff will help you fill it out during the physical examination.

2. If you don't have a 2-inch normal color photo, you can use a passport photo or a 50×50 photo.

On the day of picking up the results and packaging, you must bring:

2. Letter of appointment.

3. All documents listed in the hospital notice.

Note: From Jun. 5438+Oct. 2009 10, notaries need not affix their seals to the hospital.
