Some tendons are covered by fibrous tissue sheaths called tendon sheaths. Its function is to facilitate tendon sliding and make fingers flex and stretch normally.
When the hand is moving, the tendon slides in the tendon sheath, and some tendon sheaths gradually thicken and narrow due to this friction. Causes tendon movement to be blocked and local pain occurs. This condition is called tenosynovitis, also known as stenosing tenosynovitis.
The most common part of tendon sheath thickening and stenosis is the starting point of flexor tendon sheath of each finger, which is equivalent to the distal palmprint and tendon sheath located at the styloid process of radius. Tenosynovitis of flexor occurs in the aforementioned position, while tenosynovitis of styloid process of radius occurs in the latter. Both are very common.
Tenosynovitis of flexor tendon is most common in the tendon sheath of flexor tendon of thumb, food and middle finger, and rarely occurs in those without multi-finger and little finger. After the onset, patients feel:
① Finger flexion and extension disorders, especially in the morning, will be improved after more activities. ② There is tenderness and induration locally. When the flexion surface of the metacarpophalangeal joint of the affected finger is pressed, tenderness and induration will occur. The induration is the thickened part of the sheath. (3) In severe cases, it can produce a click sound, that is, when the affected finger moves, the muscle position makes a click sound through the tendon sheath at the narrow part. Tenosynovitis in this case is also called "snapping fingers". When the hand touches the induration and moves the affected finger, this feeling of snapping is more obvious, and sometimes the affected finger snaps. Some fingers bend but can't stretch or stretch and can't bend, so they need to stop to help bend and stretch. This situation is called locking.
Tenosynovitis of styloid process of radius, there is a bony process on the radial side of wrist (that is, thumb side), which is called styloid process of radius. There is a tendon sheath with two tendons (extensor pollicis brevis tendon and abductor pollicis longus tendon) passing through it. Due to more activities of thumb and wrist, tendon sheath is often inflamed; It is called tenosynovitis of styloid process of radius.
After the onset, patients feel:
① The styloid process of radius is painful and swollen.
② The thumb is inconvenient to move, especially in the morning, with occasional snapping. During the examination, the styloid process of radius has obvious tenderness, and sometimes induration can be felt. The patient is required to bend his thumb in the palm of his hand with his fist, and then passively tilt his fist to the little finger side. If there is pain around the styloid process of bone, it means tenosynovitis exists.
The treatment of this disease is intrathecal injection of hydrocortisone acetate or triamcinolone acetonide once a week/kloc-0 times. To relieve the pain during injection, it can be mixed with 1% procaine or lidocaine hydrochloride for injection. Generally, it can be cured after 4 ~ 6 injections. It has a good curative effect on early tenosynovitis. Acupuncture, massage, Chinese herbal medicine and hand fixation can also be used, all of which have certain curative effects. If the above treatment fails, surgical treatment can be adopted, that is, cutting the narrow tendon sheath under aseptic conditions, which is called healthy sheath incision. The surgical effect is good and recurrence is not easy.
